    The Saturday Team Trial Meet(10/26) has a new start time of 5:30 PM, and swimmers are expected to arrive by 4:00 PM. It's a small meet and expected to go pretty fast.

    If you do not have a printer, please pick up the documents at the FCHS Front Desk.

    Forsyth Central 25-26 Physical Form (coming soon) - includes the forms listed below:

    • Forsyth Central Physical Permission Form
    • Permission to Participate and Transportation
    • Student and Parent Concussion Awareness Form
    • Student and Parent Sudden Cardiac Arrest Awareness Form
    • PreParticipation History & Physical Evaluation Form - Doctor signature must be after February 15, 2024 or a second physical will be required if the student qualifies for the State Meet

    YEAR ROUND SWIMMERS -WEEKLY ATTENDANCE FORM 

    **At the end of the season, each club swimmer should have at least 12 sheets turned in to Coach Bell. This does not include holiday practices as it is expected that you are in attendance at either club or high school practice.**
